Intoxication by Digitalis Purpurea in Suicidal Intention – a Case Report

چکیده
We report a case of a 27 year old female with a known progressive major depression, who ingested an unknown amount of leaves of Digitalis purpurea (Figure 1) for attempted suicide. The patient developed nausea, emesis and a reduction of color vision. Furthermore, the ECG showed atrioventricular block II° (Wenckebach, Mobitz) with the necessity of pacemaker implantation. The therapeutic range of digitalis compounds are narrowed by the toxic side effects due to interindividual variability of the pharmacologic impact and may be accompanied by potentially life-threatening outcomes. The intoxication by Digitalis purpurea is a potentially life-threatening serious adverse event and requires an ICU monitoring. The diagnosis of intoxication by digitalis derivates is a demanding issue due to the various and often unspecific clinical symptoms and ECG manifestations. The underlying diseases, however, may lead to comparable symptoms and ECG manifestations without the possibility of prompt distinction. Due to the different composition of glycosides, their different half-time values and different cross-reactivity features to common assays, the clinical course, the extent of the severity of symptoms and long-term ECG monitoring represent the adequate method of surveillance and treatment control.

Bidirectional Tachycardia after an Acute Intravenous Administration of Digitalis for a Suicidal Gesture
Acute digoxin intoxication is a life-threating condition associated with severe cardiotoxicity. Female gender, age, low lean body mass, hypertension, and renal insufficiency may worsen the prognosis. Arrhythmias caused by digitalis glycosides are characterized by an increased automaticity coupled with concomitant conduction delay. Bidirectional tachycardia is pathognomonic of digoxin intoxicati...
متن کاملQuality Assessment of Serially Ultradiluted and Agitated Drug Digitalis purpurea by Emission Spectroscopy and Clinical Analysis of Its Effect on the Heart Rate of Indian Bufo melanostictus
The investigation of ultradiluted (homeopathic) drugs is extremely interesting and challenging, and from that point of view this study shows novelty. A study of in vivo changes in heart rate of the Indian Bufo melanostictus caused by commercially available serially ultra-diluted and agitated extract of Digitalis purpurea has been tried in order to understand their pharmacological role. RR inter...
متن کامل[Suicidal digitalis poisoning: considerations concerning treatment strategy with antibodies].
Suicidal digitalis poisoning is life-threatening and often has a fatal outcome. The treatment and clinical course of acute poisoning with 250 mg digitoxin in a depressive male patient aged 48 years are reported. Marked elevation of serum digitoxin level (360 nmol/l 8 hours after ingestion) and initial hyperkalemia (5.7 mmol/l) as well as the history of excessive dose intake, pointed toward seve...

Foxglove (Digitalis sp.), as an ornamental and medicinal plant, is used for treatment of heart congestive disorders. Use of plant cell and tissue culture can be effective in large scale micropropagation and stable production of medicinal compounds. In this study direct and indirect regeneration methods in two species of this genus (D. purpurea and D. lanata) were investigated in response to dif...
